I lost all of my electrical toys, dildos, and numerous other items due to flooding. Do NOT send me this or any checklist in any form unless you intend to request a session for the near future.

There are three ways to utilize my checklist.

  1. Fill out and send the online form.
  2. Download the zipped text file, unzip, and paste into either the body of an email or a word processor. Be sure to save as .TXT and not as a .doc or any other format.
  3. Either "Save Page As" (CTRL S) or copy and paste the questions from the appropriate page.
